Weekly rate: $2,127.20

About the role

An exciting opportunity exists for a Registered Nurse to provide and coordinate integrated services for clients with complex health care requirements.

The role provides information, support and care coordination to people with long term complex health problems.  There is a strong focus on supporting client self-management to prevent unnecessary admissions to hospital and presentations to the Emergency Department.

Depending on the position the successful applicant will hold, or be able to obtain, a Police Criminal Check, employee Victorian Working with Children Check and be willing to undertake a Disability Worker Exclusion Scheme check.  Current immunisation records/evidence is required.
