About the role

We have an exciting opportunity to join our Primary and Preventative Health team in the Discharge Support Service department in a Casual  capacity. Discharge Support Coordination is a vital and pivotal role working across acute and community services.

The aim of the role is to achieve a smooth transition for clients/carers referred to the Discharge Support Service with complex discharge needs, from an acute episode of ill health, back to the community. 

The discharge support care coordinators facilitate appropriate support services for clients referred to the Discharge Support Service, and act as a broker in the purchase of those services, for those clients identified as requiring Short Term Support services. 

 

To be successful in this position a great level of teamwork is required with multidisciplinary team members and external service providers.

The successful candidate must have or obtain a National Criminal Police Check. Depending on the role, a Working with Children Check Clearance, Disability Worker Exclusion Scheme check, and/or NDIS Worker Screening Check may also be required.
