About the role

An exciting opportunity exists for a Registered Nurse to provide and coordinate integrated services for clients with complex health care requirements. The role provides information, support and care coordination to people with complex health problems. There is a strong focus on supporting client self-management to prevent unnecessary admissions to hospital and presentations to the Emergency Department. Remote patient monitoring and innovative health options with a capacity to undertake research is promoted.

  • 5 years post grad experience
