Home Care Packages - Care Manager 

Permanent Part Time (64 hours per fortnight)

About the role

 An exciting opportunity exists for a Care Manager to join our rapidly expanding team.

If you can offer the highest standard of care to our consumers and have good time management and organisation skills, we would like to hear from you. Reporting to the Aged Care Services Manager, the Care Manager is accountable for overall performance and service delivery of our Home Care Package care recipients.

Within a Consumer Directed Care framework, you will manage care and services to enable them to continue living safely and independently at home.

 

You will bring:

  • Preferred but not essential -Degree, Diploma or Certificate in a Community, Health or Management discipline
  • Previous experience in a case management role will be highly regarded
  • Advanced competence in management information systems and software packages, such as Microsoft Office
  • Proven ability to negotiate, manage and operate within a budget
  • Excellent communication skills and a genuine care for older people
  • Flexibility and self-motivation

The successful candidate must have or obtain a National Criminal Police Check. Depending on the role, a Working with Children Check Clearance, Disability Worker Exclusion Scheme check, and/or NDIS Worker Screening Check may also be required. All employees must comply with WDHS vaccination requirements, including COVID and Booster shots. WDHS is a smoke-free workplace.
